Patient Screening prior to office visit.

Please check your temperature the morning of your appointment. If your temperature is over 100.4 , have significant cough, flu- like symptoms such as body aches, headache weakness, fatigue, loss of taste or smell PLEASE call the office 207-888-1779 to cancel your appointment we will be happy to reschedule your appointment.

 

Potential exposure?- Please call to cancel your appointment and follow testing and quarantine guidelines per your PCP.

Recent Travel?- Anyone who has travelled domestically or internationally within 14 days of your visit please notify Mindful Roots.

Please come to your appointment alone.
